Submit a Purchase Order

Please send an inquiry to Jill at speechandsmile@gmail.com for a W9 to enter in your vendor system. Please email a signed copy of the purchase order, tax exemption form and questions to Jill at speechandsmile@gmail.com.

Please note that purchase orders must have an official purchase order number, email address and an authorized signature (Party responsible for billing). 

*We do not accept purchase orders by phone. We require the information via email.
